Paid Time Off (PTO)
New full-time regular employees will earn Paid Time Off (PTO) with their first full pay period. This time may be used for vacation, sick, or other personal reasons. Full-time regular employees will accrue PTO according to the following schedule based on their employment anniversary date:

Holidays, wellness, and personal cultural days
LifeWorks NW outpatient sites close to observe the following holidays. We provide these eight paid holidays to all benefited employees.

- July 4th
- Labor Day
- Thanksgiving Day
- Day after Thanksgiving
- Christmas Day
In addition to the above holidays, LifeWorks NW offers two personal cultural days and four wellness days over the course of the year.
Extended Illness Bank (EIB)
Full-time regular employees will accrue 56.16 Extended Illness Bank hours/year. When an employee has been ill for three consecutive days, they may access their EIB for time available beginning on the fourth day of absence. An employee who has been hospitalized or is eligible for Family Leave may access time from their EIB immediately.
Unpaid personal leave
LifeWorks NW unpaid personal leave policy allows regular, benefits-eligible employees (with a minimum of five years of service) to take time off for personal needs and interests in excess of that which is available from their Paid Time Off benefit, with management approval. The amount of unpaid personal leave an employee is eligible to request is based on the employees’ years of continuous service, as shown in the schedule below:

Retirement
As an employee of LifeWorks NW, you will be eligible to participate in a 403(b) Tax Sheltered Mutual Fund. When benefited employees have been with LifeWorks NW for over a year, they are eligible for LifeWorks NW’s contributions based on a percentage of regular salary, as shown below:

Vesting
Employees are fully vested in employer match funds when hired. Employer match contributions begin the second year of employment.
Insurance Coverage
LifeWorks NW offers regular full-time employees (40 hours/week) coverage for medical, dental, vision for an employee contribution of $100 per month. Life and long-term disability insurance is available at no cost, with optional voluntary coverage.
LifeWorks NW offers pro-rated benefits for part-time employees (20-39 hours/week). Employees may elect coverage for dependents and domestic partners at the premium cost.
