Hazelwood & Horizon

Hazelwood & Horizon are residential treatment homes for adult men living with a Serious and Persistent Mental Illness (SPMI), such as schizophrenia or bipolar disorder. The homes provide a stable, supportive environment where residents can focus on treatment, daily living skills, and reconnecting with the community at a pace that fits their needs. The program typically serves five men at a time.
