Below are additional forms and information you or your client may find helpful when establishing care at LifeWorks NW.

  • To request a client’s medical record, please FAX a cover sheet detailing the requested documentation, along with a HIPAA-compliant Release of Information form to 503-645-6010. For questions, contact us at 503-645-3581 ext. 2701.
  • For questions about the status of a DMV Certificate of Completion, call your clinician or our Addictions Reporting Coordinator at 971-727-8026 ext. 3144.
