Transition-Age Youth (TAY)
If you are between the age of 16 and 24 with significant mental health needs, our Transition-Age Youth (TAY) program may be a good fit. We work in the community (at your home, at a coffee shop, at your school) to build up your mental health and help you achieve goals like housing, education, and employment.
TAY services are for teens and young adults who have been:
- Experiencing a mental health challenge that makes everyday life hard.
- Living in a residential or supported environment.
- Admitted to the hospital for mental health services.
- Having difficulties feeling connected in mental health treatment.
- Involved in the criminal justice system.
- Struggling to control drug or alcohol use.
If you are interested in the program, you can expect to:
- Meet once or twice each week with a therapist.
- Work with a Supported Employment or Education Specialist, if desired. You have access to job training and guidance.
- See your prescriber regularly for medication management.
- Attend monthly activity groups with others who may have similar experiences as you.
